Athletic Clearance (Online)
Online Athletic Participation Forms, (available online at www.homecampus.com , including a physical exam, are required to try out for all MHS sports. If you are planning on trying out for multiple sports during the school year, you must complete athletic clearance forms for each sport.
Eligibility
Students must maintain an un-weighted 2.0 or higher GPA both scholastically and in citizenship (Advisory citizenship grade does count towards citizenship GPA) based on the most current grading period to remain eligible for sports participation.
Sports Physicals
San Diego Unified School District requires that every student/athlete have an annual examination. Physical exams are good for one calendar year.
Physicals must be completed every year dated June 1st or later.

2025-26 Athletics Physical Examination Forms
Students participating in extra-curricular activities, including athletics, are strongly encouraged to purchase an ASB card (cost $20) and be eligible to participate (see www.cifsds.org for complete eligibility information and the CIFSDS calendar). If you are interested in a particular sport, you should check this Athletics website or contact the coach prior to the season for information about tryouts.
